EDC will provide all necessary materials for your blood collection and delivery to the testing laboratory. All we require from you is an order or requisition form from your primary care provider (PCP) indicating the specific tests to be performed. This information helps us determine the appropriate tubes needed for your blood draw. If you wish to book an appointment for two people at the same location and time, only one appointment slot is needed. Please include both names in the booking notesit
