When scheduling mobile appointments, please fill out the form completely, including the address where you would like the services to be provided in the notes section. A representative will contact you to confirm your appointment and provide payment details. 

Customers with multiple kits may incur additional charges, but they will receive a  discount for same-day services as a token of appreciation for using our services.

Accepted payment methods include Credit, Debit, HSA, and FSA. In the event your FSA or HSA is unable to process the payment, please have an alternative payment method on hand.
